I'm so glad you're interested in working together! Please answer these quick questions to confirm your initial eligibility for Fertility Foundations.

1. Are you 18 years of age or older?
2. Are you a California resident, and will you be physically located in California during our virtual consultations?
3. Do you understand that my services provide nursing education and coaching, not medical diagnosis or treatment?
4. Are you willing to complete intake paperwork and baseline laboratory testing before your initial consultation?
5. If you're trying to conceive with a sperm-producing partner, are you willing to arrange a semen analysis separately, at an additional cost?

All new clients begin with Fertility Foundations. Applications are reviewed before enrollment. No payment is required at this stage.
